To re-order a prescription, simply fill out the form and press the Request Prescription button. After pressing the Request Prescription button a confirmation page with all of your prescription information will be loaded.

Please allow 24 to 48 hours for our office to respond to your request. We will send you an email or call you to confirm the refill of your prescription.
